With the rise of LLMs, it became possible to not just transcribe a medical conversation but to organize it into a structured patient note, turning one of the most tedious parts of a clinician’s day into something close to effortless. So I built an app to do exactly that.

The design problem
The hard part isn’t the transcription. It’s trust. A clinical note has a specific structure, and a clinician needs to glance at the output and immediately see that it’s right, then fix it fast when it isn’t. Most of the design work went into turning a raw, messy transcript into a clean, editable note that maps to how clinicians already think. I designed it to work wherever a visit happens: capture on the phone or Apple Watch in the room, then review and edit on the web afterward.
